Before You Book

• HMO Insurance: You must have a referral/authorization from your primary doctor before your visit. Without this, we cannot see you. Check our "Accepted Insurance" page if you aren't sure about your plan.

How to Schedule

Our phone lines are handled by an AI receptionist. It is designed to quickly guide you to the right person or service based on what you need.

Option 1: Book Online (Recommended)

1. Select a Time: Click our booking link and pick a time that works for you.

2. Enter Details: Provide your name, birthdate, and contact info.

3. Submit Request: Tell us why you’re coming in and click "Request Appointment."

4. Wait for Approval: Your request is pending until our team reviews it (typically 1-3 business days). You will receive a confirmation email once approved.

Option 2: Book by Phone

When you call, our AI assistant will send a booking link directly to your phone via text. Follow the "Book Online" steps above to finish your request.

Preparing for Your Visit

Save 15 minutes at the office by completing your paperwork early:

• Digital Check-In (Recommended): After your appointment is confirmed, we will send you an email and text containing a link to our digital forms

• Paper Forms: Prefer pen and paper? [Download and print our intake forms here] to bring with you.

Cancellations

• Cancellations: Please provide at least 24 hours' notice if you need to reschedule or cancel or a cancellation fee may be charged. 

What to Bring

To help us provide the best care, please have the following ready:

• ID & Insurance: Photo ID and your insurance card (digital or paper).

• Medical Records: Any doctor’s notes, X-rays, MRIs, or CT scans related to your visit.

• Health History: A list of your current medications, allergies, past surgeries, and any doctor referrals.

Arriving at the Office

1. Sign In: Please write your name and arrival time on the sheet at the front desk.

2. Paperwork:  If you finished your forms online (or brought them printed): You’re all set! Just have a seat until we call you back.

   • If you still need to complete forms: We will provide an iPad or paper forms. Please arrive 15 minutes early to ensure you have enough time to finish before your appointment.

During & After Your Appointment

• The Exam: Once you’re checked in, the provider will bring you back to a private room to take your vitals and discuss your health concerns.

• Next Steps: Before you leave, we will review your treatment plan and any prescriptions.

• Scheduling: If you need a follow-up, we will book it right then and give you a business card with your next date and time.
