What to Bring
To help us provide the best care, please have the following ready:
ID & Insurance: Photo ID and your insurance card (digital or paper).
Medical Records: Any doctor’s notes, X-rays, MRIs, or CT scans related to your visit.
Health History: A list of your current medications, allergies, past surgeries, and any doctor referrals.
Arriving at the Office
Sign In: Please write your name and arrival time on the sheet at the front desk.
Paperwork: If you finished your forms online (or brought them printed): You’re all set! Just have a seat until we call you back.
If you still need to complete forms: We will provide an iPad or paper forms. Please arrive 15 minutes early to ensure you have enough time to finish before your appointment.
During & After Your Appointment
The Exam: Once you’re checked in, the provider will bring you back to a private room to take your vitals and discuss your health concerns.
Next Steps: Before you leave, we will review your treatment plan and any prescriptions.
Scheduling: If you need a follow-up, we will book it right then and give you a business card with your next date and time.
